A 72-year-old male was admitted for abdominal pain with elevation of amylase and lipase. At CT-scan was dilation of the main pancreatic duct. MRCP revealed mixed type diffuse IPMN with suspected pancreas divisum. EUS confirmed IPMN. Pancreatoscopy showed diffuse papillary intraductal aspect. Target biopsy confirmed IPMN with dysplasia.
